As a student of both Erotica and Horror genres I have become frightfully aware of the fact that no matter how good the writing is, if the novel even smacks of somethings readers are not willing to accept as part of life they will not read. The fact is that life is not about feel-good, it's about conflict and resolution. Few lives begin at birth and end at death without drama which in many cases involves some sort of horrific event, in which sex is often involved, either before, after, or during said event.
Many novels in the romance era are set in real-time with real people for this reason. It is easier to imagine that everybody in history lived happily ever after when in reality this was not so. There was as much drama and treachery in the lives of King, Queens and nobles, perhaps more so, as there in the rest of society. We just hear more about them than the average citizen.
Today we live in a world where treachery and drama are daily experience in conflicting lifestyles with unhappy endings.
