You should probably put your name and pen name in a cover letter attached to the manuscript. A cover letter has all of your contact information and so forth in it. If you search for "manuscript cover letter" or "manuscript query letter" you will find what you're looking for.
There are lots of publishing resources in the "Invalid Item" too. If you join, there is a whole book of information to help you get started... how to find publishers, how to write a query letter to an agent or editor, etc.
