If you mean that you successfully added the chapter, but that later it was missing, then the most likely explanation is that the owner of the story deleted it, either that chapter itself or one that preceded it in the story line, or they deleted the whole item, or they deleted or abandoned their account.

You should always check that your submission results in a success message.

The site recommends keeping a copy of what you submit on your own computer to prevent loss of your work in such situations, and because problems can occur when submitting online.
