Yours is already set to ON. You can check that by viewing your Reviews tab.

To open the manage area of the Reviews tab, click on the gear icon at the top right.

To turn review requests OFF, use the top setting in the drop-menu labeled:
          Minimum Gift Points accepted for Review Requests:
[To turn it back on, use one of the GP amounts.]

To get listed on the Request A Review page, fill out the Review Style section.

Click Submit Review Info to save the changes.
