Mentioning the black death is okay at an E rating. It's a disease.

However, a piece of fiction that features the black death is not likely to merit an E-rating.

Mention medicine, and the rating requires at least ASR, but that's for the medicine, not for the disease itself.

Get graphic about the description, probably higher, but that's for being graphic, not for the disease itself.

Use the disease to commit one murder, 13+. For committing murder, not for the disease itself.

Mass/multiple murder is at least 18+. It goes higher if the description is more graphic than 18+ allows for, or if other elements that specify a higher minimum rating are included.
