Greets,

No, when you click the picture it brings you into the item's normal shop page.

Small is the default setting no matter where you click into an item's page.

For example, when you click below, you'll see the item's shop page.
The default setting is "small":



The above link is not related to your wishlist, yet still shows the same small.

The only way someone can order a product from your wishlist is to use the "Add to Cart" directly within your wishlist.
If they click into the item, they're no longer on your wishlist and can't wish-list shop for you.

Hope this clarifies and sorry for any confusion, but thanks for confirming there is no issue with the wishlist system itself.
