Standard Manuscript Format is a best practice. You can google the specs, it's not hard.
If you skip the address and phone, you still need your name and email. Think about it, you're about to hand over your hard work without ANY identification that it's yours?
For beta-reading, you do want to lock that down a bit from Publishing.
use Google Docs and the Share feature with Comment-only set for the individual I share with.
Also make sure the footer includes a confidentiality notice.
