The contest entry issue is probably because a previous month's entries weren't cleared before this month's round began.

You can check your survey submissions at My Account/Survey Responses for the contest to see if there's a previous one, and what month it was.

For the other issues, the URLs in the error messages look odd.
When you name the file for your cover, does the file name contain more than one period right before the file type? [spaces and arrows added]

I'd also suggest you clear your browser cache to get rid of any residual problems:
Log out. Clear browser cache. Shut down all browser windows. Reboot the browser.
