Toward the end of episode 14 there's a very clear shot of said plate. I didn't see it the first time, but I saw it the second time.

As for good ol' Kaito, I'd say that he will come back at least in episode eight, when they do the flashback to explain what brought everyone to this point. With his power not directly stated, it is possible that he will reappear in the current timeline. Many suspect he's immortal, but after this episode, I wouldn't be surprised if he's able to stay alive through some astral projection or putting his real self elsewhere. Does that make sense?
