More of a bug fix then a suggestion, but here it is.

I understand the need to use r-item is scroll. However, the purpose of the double brackets seems to be so that the proper code can be seen,

I've seen it countless times that people use the double brackets and b-item, or simply item, and it gets converted into r-item. I simply ask that you add exceptions if double brackets are used. An issue might be that the text is changed before it's even recognized as a double bracket.
