This button opens the Review Tool in a pop-up. Sounds like that part works okay for you.
The template manager drop menu doesn't have any choices for you until you create one. That's the New button on that bar. Clicking that opens a new pop-up window where you can create/edit the template portion of a review (i.e., boilerplate) and save it. Then you need to close both pop-ups and reload the Review Tool. This time, there will be a template listed in the menu. You select the one you want and use the Load button to add it to the review edit space.
Note that Load wipes out anything that's currently in the review edit space, so be sure to load the template before typing anything.
