The old saying, KISS, 'keep it simple stupid'. Not to say you are in any way simple or stupid. But I think your story has so many nuances. it may be hard to get a handle on it all right now.

What about doing some 'stream of consciousness' writing? Write as your character. Become Ruth for a day. See what happens.
