Okay, I had a story Idea I was gonna put in the previous chapter but you snooze you lose. This suggestion is set in a version of season 4 where Marinette and Adrien are 18 years old:

A student in one of the other classes at Marinette's school is pregnant. Unfortunately several female students have been mocking her; the weight she'd gained, her massive belly, her mood swings, her cravings, the fact that her boobs (which were now bigger than theirs) were leaky.

Inevitably this drew the attention of Shadow Moth, who Akumatized her into a super-villain with the power to transform women into an imitation of pregnancy (men were unaffected), which she did eagerly. He also sent a sentimonster; a copy of her personal hero' Chat Noir.

The real Chat Noir soon arrived with Ladybug but not only did Fake! Noir keep him occupied enough for Babyfaker to hit Ladybug, it also destroyed the lucky charm, meaning that when the heroes defeated Babyfaker and destroyed Fake! Noir, Ladybug was unable to perform Miraculous Ladybug and reverse the transformations, leaving her and several other girls permanently trapped in an imitation of Pregnancy.

Or maybe you have a different Idea about how an Akuma can inflict PFP on Marinette, but is it just a random foe of the week, or an explicit plan by Hawk/Shadow Moth to make Ladybug appear permanently pregnant.
