There's should and then there's practical. SM's usual response for accidental events is to ignore such things -- you get to keep any bonus resulting from it. The cost in the time it would take to "clean it up" is higher than whatever bonus might occur. Duplicates happen with double-clicks occasionally, too.
For future reference, in light of the 5 chapters, if a review submit seems to fail but without an error message, check your existing review count, load a fresh page, and see if your review count went up by one. You could also check the View Your Review or your Given Reviews (the private listing) to see if it went through, though if the servers are really backed up and haven't updated the database, that might not show it to you in a timely fashion.
