Open the item. Click the gear icon at the upper right corner of the item. Click on Delete Item at the bottom of the menu. Confirm the deletion. This moves the item to your Recycle Bin.

Once you're sure you want to permanently delete the item, such as to reclaim the item slot in your portfolio, scroll to the bottom of your Portfolio and open the Recycle Bin. Click the option to Purge Bin, and confirm. At that point the item will be permanently removed.
