Greets, From your "My Responses" page found under the "My Account" pulldown, you'll see that the entry was successfully submitted and you can view it. Sometimes a duplicate click gets through on the survey submission system. As for the other error(s), these seem to be isolated to your account, so we'll just have to try to work through it with you. In particular in your account, I see "Tempus fugit" is throwing off a lot of errors because of the "Time.docx_html_62cb737.jpg" linked in the item. It looks like this is because you tried to drag and drop in a document with an embedded image. The system does not correctly process the image, so you're left with a broken link in the item. Aside from your survey error tonight, the inability to load the image is the only error I see in your account since 10/5/2021, and it's there around 50 times since that date.
