Speaking from my dabblings in the publishing world...
If it is going to lead directly into the action, italics. If it is a set-up, separate section and no italics. If it is the first if many dreams, have them all in italics.
At least 2 publishers I've worked with don't like dream openings, while the rest don't seem to care so long as it feeds onto the story properly.
