I can only speak from my very limited experience of writing my first novel, which is a Work in Progress right now.
Although I've created stories in my head for years and years (all character-based, very short, soon forgotten, and never very involved with regards to plot and development), I never actually sat down to write a book until a theme hit me. I then created characters and a plot to match that theme (well, to be frank, I think the characters found ME and told me their plot, but that's a whole 'nother ball of wax). Since I don't have any other experiences to draw from, I can only say Theme First, then everything else follows. I don't think that's the way it *has* to me, mind you, but it's the way it has been for me, at least up to now. Guess we'll see what the future holds.
