Johny was normal boy, he was about 16 years old. His life was normal, maybe little boring. Until one day. That day he was on vacation in China. He was camping in some recreational area. That afternoon, when he wanted to make lunch for self, he taked water from nearby spring. (Fans of Ranma maybe recognised that Johny misunderstooded recreational area with training area Jusenkyo. Yes, he is there...)
He used water from Nyannīchuan - After he ate that food, he started feeling weird. The curse started to affecting him. But because he wasn't fall into spring and/or maybe because he boiled that water before he got to contact with it, the curse got new form. At first, his changes will happen randomly, without contacting with water and same it is with changes back. Also he can change completelly or partially, so he can become boy with breasts or he can change into dickgirl. And his girl appearance can be always different. One times he can become short-haired brunette with B-cups and next time he will be long-haired blond with C-cups. But everytime he transform into nice girl, never to ugly.
Sex is allowed, pair him with male or female, it depends on you. But keep him with nice looking persons.
