Ever heard why a product had to change it's user instructions?

For example:

A phone operator at a baking company (aka Betty Crocker) receives a phone call on the help line from a very irritated man:

"Hi, may I help you?"
"YEAH! I followed the box's exact instructions on baking this cake and it turned out awful!"
"What happened Sir?"
"The oven burned the whole pan and then the cake would not come out of the pan! The whole cake is RUINED!"
"The cake wouldn't come out? Did you butter and flour the bottom of the pan as the instruction said?"
"YES! But the butter and flour started a drip grease fire in the oven and the cake got burned! Then I couldn't get it out of the pan!"
"Uhh, where did you butter it sir?"
"All over the bottom just like the box said to!"

So now the baking company changed the instructions to "Lightly butter the INSIDE of the pan..."
