Wow! I'm not usually a big fan of poetry but this really touched my heart. I definitely got the picture on how you feel about someone and how they made you feel. I want to know more about this person and what they did specifically to make you lose yourself. That's just me though. I'm a curious and rather nosy person. The only line I didn't like was 'I view life through the twisted glass of a day dream periscope'. Not because it isn't beautiful. It is. But because the sentence itself is a bit long. All of your other lines are short and to the point. This is just my opinion. I truly loved this piece. Keep writing and I will keep reading!
