I really like the motivation for this survey. It's a good idea to learn about the kind of characters that readers want to read about, and this survey considers multiple aspects of a reader's personality in order to figure these things out. However, a couple of the questions contained some inherent assumptions that made it difficult to answer them. As an example, simply because an individual thinks that it is a good idea to include religion in stories, does not mean that they only want their own religion included, or even that they have their own religion. But overall, I think it's a very good idea to be asking these kinds of questions.
