Yes, it is often too complicated to see the extended metaphor which is why I like this one. Seems clear to me to see the spiritual themes in the Hospital Window.
Extended metaphor is afterall simply an series of related imagery. You do this quite often yourself, Alfred, even if you don't realize it.
I agree with Joseph, it IS often difficult for the average reader to see writing from the writer's point of view. Of course, that why one takes an English Lit class, or specializes in it as you he done. to learn to see how good writing works in order to improve one's own skills.
