I seldom do the basic 250 character review though I am not sure they exceed 1000 characters. I just write what I think is nesessary to encourage, suggest, and thank the author for what I got out of the piece.
I will admit that if I really didn’t enjoy the piece or if it wasn’t a genre that I know much about (I get a lot of review requests) then I find an excuse to not submit a review.
I have a couple of templates, both long and short. I use them based on the piece and what I want to convey. Some authors lately have asked for specific feedback. In those cases, I provide the answers to the questions they have from my POV.
My templates are stored on my tablet and on wdc. I like to have the template open and the piece I am reviewing in separate tabs so I can flip back and forth.
Visually appealing? Nah - just basic bolding for the headlines on the topics. On rare occasions I use dropboxes.
