Presentation is a key part of writing as errors can be very offputting, and can mess with comprehension too. So I always proof-read my reviews before sending them. Sometimes a typo might slip through if I haven't been ultra-careful.
I do comment on grammar and punctuation because it is an integral part of the piece. My grammar knowledge is pretty good, so I feel reasonably confident, but if I am not sure about something, I'll check it as sometimes what seem like errors are just differences between US and UK spellings and grammar.
As for content, I would fact check something I wasn't sure about, or mark it as an area the writer might like to check. But generally any comments I make on content are opinion so fact checking isn't really relevant. If I make a suggestion like using metaphors, I will usually include an example, or put in a link to further info.
