My happy medium is if there are line by line errors, I will only mention a couple and recommend to the author to re-read their story to find other similar errors.

Telling someone that their story is fantastic is nice. But the author, in my opinion, will want to know what makes if fantastic. I know I would. So share what you thought was fantastic.

I try to review on what I would want to be reviewed on, unless the author has specified certain aspects they want reviewed. Then, I stick to what they requested.
