So each participant starts off with a blank journal/notebook. They start it off, either with a simple set of instructions or with their own however-long introduction. Then the book is sent to each participant in turn for them to add stuff to it. Eventually it comes back to the original owner full of writing, photos, stickers, postcards, recipes, quotes, whatever. And because each participant starts with a journal and it goes around the circle, each participant finishes with a (slightly different) completed journal to keep.
