I love the childlike simplicity of your poem's opening: it feels like a goofy pop song, but then the sincerity of feeling develops and we see the depth behind the ordinary words. It doesn't take fancy vocabulary to show someone how much we care, and how they've changed us for the better.
This could, indeed, be a song; it has the symmetry and balance of some of the best songs. My favorite verse is the fourth, where the narrator describes how "their cup runneth over" with their loved one's blessings of hope and care. If you have someone like this in your life, maybe you could write this out on paper and give it to them, or make a pretty font styling of it on a Word doc and print it out.
I would like to point out, gently, that you do need to be more aware of the genres. Romance, Relationship, Family, Emotional, Philosophy, or Inspirational would work here, and give the item greater visibility when browsing. Also, you can probably rate it E without anyone objecting. Love is universal and comes at any age
